IMPORTANT REMINDERS:
• LCISD pre-k is open to students who qualify under one or more of the district's eligibility requirements, found here.
• This event is for parents only; students will be welcomed by campuses at a later date!
• If you are an LCISD employee who has signed your child up for pre-k, you do not need to attend Pre-k Registration.
• Please enter through Door 5 on the SOUTH side of the LCISD Administrative Offices (the door nearest the LCISD Police Department). Parking is available on the south side of the building, near Door 5.
• Please present all required paperwork:
- Proof of residency
- Child's birth certificate
- Child's Social Security card
- Child's current shot record (see below for specific record requirements)
- Proof of income; TANF or SNAP letter
- Parent/guardian's driver's license
You cannot register your child until you are able to provide ALL documentation (see specific vaccine record requirements below).
• If you are unable to attend today's event, or you do not have all of the required documentation available today, don't worry! You can register your child on campus beginning May 1!

IMPORTANT: ENROLLMENT VACCINATION REQUIREMENTS
Students who are four years of age or older are required to have updated vaccination records upon enrollment. Listed below are required vaccines for all students who are at least four years old. A student cannot enroll in pre-k or kindergarten without proof of the following vaccines:
- Diphtheria, tetanus, and whooping cough (pertussis) (DTaP) (fifth dose)
- Polio (IPV) (fourth dose)
- Measles, mumps, rubella (MMR) (second dose)
- Chickenpox (Varicella) (second dose)
You must provide your child's OFFICIAL VACCINE RECORD for enrollment. Print-outs from a patient portal will not be accepted!
An OFFICIAL VACCINE RECORD INCLUDES:
• Documentation of vaccines administered that include the signature or stamp of the physician or physician's designee, or public health personnel, is acceptable. Immunization records generated from electronic health record systems must include clinic contact information and the provider's signature/stamp.
• An official immunization record generated from a state or local health authority is acceptable.
• An official record received from school officials, including a record from another state, is acceptable.

As we near the end of the school year, remember that all students planning to participate in 6th grade pre-athletics, all 7th-12th grade athletes, and all 9th-12th grade marching band members MUST have a new physical on file to participate in these activities in the 2023-24 school year! Due to poor road conditions, LCISD buses will run on pavement and caliche roads only for morning routes tomorrow, Tuesday, April 11. Bus riders who live on dirt roads will need alternate transportation to school.
